Here's one live, but it's TV and not concert:



The only other live version I know of was performed on Saturday Night Live, but I think the above one is better. I don't know if she's ever done it in concert, but I'm sure someone here will know. If not, then she definitely should have. I can't imagine why she wouldn't have, but stranger things have happened. If anyone has an audio of the above live version, please post it. It's really beautiful. I like it better than the album version.
