For one thing, I don't think Mama is walking down too many streets- I suspect a driver is waiting outside for her at every stop. Secondly, she was obviously incognito a la Mrs. Buckingham- with the black beret and (presumably) sunglasses. She probably took the sunglasses off when she was inside trying on jewelry and the photog clearly espied her through the shop window. I think the fact that she has a ruddy complexion is nothing new. But those marionette lines are something else, perhaps magnified under the glare of harsh indoor retail lighting. I don't think it would be an awful thing if she had those lines fixed... after all, there are some VERY good examples of good plastic surgery- Demi Moore, Madonna, etc. It's not what it used to be...
