Unfortunately, there's no way to avoid them entirely, because they're often in that restaurant kitchen, preparing your poached eggs & corned beef hash.
I always ask the waitress to bring me my toast with butter on the side. I'd prefer to be the one to pick it up, hold it in my hand, & butter it.
